Close the risk, not just the ticket
Example: a stale configuration caused checkout failures. The change is a configuration validation gate; the owner is the platform team. Verification replays a stale configuration in a controlled test and records the rejection evidence. Shipping the gate makes the action Delivered. A passing verification test supports Verified. Document residual risk and an acceptance owner when choosing Risk accepted.
